🌟 What You'll Do
As our Discharge Planner, you'll be the bridge between care and independence. You'll help residents transition smoothly from our facility to their next destination—whether it's home, assisted living, or another care setting. Your role will include:
- Coordinating discharge plans with residents, families, and interdisciplinary teams
- Ensuring all medical, social, and logistical needs are addressed
- Communicating with external providers and community resources
- Educating residents and families on post-discharge care
- Keeping documentation accurate and timely
- A heart for helping others and a knack for organization
- Experience in discharge planning, case management, or social work (preferred)
- Strong communication and problem-solving skills
- Ability to work collaboratively with clinical teams
- Familiarity with Medicare/Medicaid and insurance processes is a plus
- Competitive hourly pay $20 per hour + /DOE
