Case Manager- CCM (R&D)



Salary Range: $ 21.50/hour - $32.25/hour
Full-Time M-F, 8-5 pm
JOB SUMMARY
Under the supervision of the Center Manager and with the support of the CCM Program Supervisor, the Case Manager will provide complex case management services to an identified population of stable seriously mentally ill (SSMI) patients who have been referred by the Primary Care Neuropsychiatry (PCN) Department clinicians. The Case Manager will facilitate psychoeducational groups and coordinate all services to achieve improved health outcomes, reduce ER visits, and with the overall goal of reducing inpatient psychiatric hospitalizations. In addition, the Case Manager will assist patients with access to mental and physical health services, social services, and referrals to various community partners.
JOB DUTIES AND RESPONSIBILITIES
• Review complex case management referrals
• Meet with patients to obtain program consent, administer required assessments and prepare results/score for LCSW review and interpretation.
• Assist patients with establishing individualized mental and physical health goals, identifying potential problems/barriers; provide health education resources.
• Provide advocacy for client and monitor client's progress.
• Coordinate logistics for mental health goal adherence.
• Facilitate weekly patient education groups for an identified group of SSMI patients. Work with clinician group to assist with treatment plans and periodic review of care plan and goals to facilitate any needed medical follow-up and/or psychosocial needs.
• Assist in timely scheduling of psychiatric and medical appointments to support patient's overall health.
• May attend various SCHC or other community based medical appointments with patients who may need additional support.
• Assist individuals with all case management activities, including but not limited to, social service referrals and application assistance for various community assistance programs and resources.
• Maintain good working relationship with community health partners by working as a liaison with other county agencies and providers.
• Document all clinical support tasks, outgoing calls and other activities to meet grant requirements which may include data entry.
• Participate in quality assurance activities to monitor and improve client service quality.
• Assist with preparing required documents and statistics for grant reporting activities.
• Participate in on-going training and reviews.
• Other duties as assigned
KNOWLEDGE, SKILLS, AND ABILITIES
· Knowledge of psychiatric disorders and terminology
· Ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines.
· Ability to use good judgment and make logical decisions.
· Strong organizational and multi-tasking skills.
· Strong written and verbal communication skills.
· Ability to effectively present information, respond to questions, and generally communicate clearly and respectfully with participants, coworkers, other agencies, and companies.
· Comfortable discussing sensitive mental and/or physical health issues with clients.
· Strong interpersonal skills.
· Ability to work with people with a variety of backgrounds and educational levels both in person and on the phone.
· Ability to work independently and as part of a team.
· Ability to travel to other offices and community locations.
· Proficiency with Microsoft Office applications including Outlook, Word, Excel and PowerPoint.
EDUCATION & EXPERIENCE
· Associate degree
· Experience facilitating patient education groups
· Minimum one year mental health or case management experience.
Experience working with mental health, alcohol and drug addicted, homeless and special needs populations.
PREFERRED QUALIFICATIONS
· Bachelor's Degree in related field
· Familiarity with social service resources in Shasta County.
· Prior experience working in case management setting.
· Prior experience working in Psychiatry or related field.
· Experience with electronic medical records
