Overview

RXCare Pharmacist- Remote- Must be located in New York for occasional onsite meetings Job Summary:
The RxCare Pharmacist will provide patient-centered services for the purpose of optimizing medication therapy to improve clinical outcomes. The RxCare Pharmacist will participate in a broad spectrum of services including, but not limited to, Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, Cost-Effective Interventions, and Transitions in Care to help achieve these outcomes.

The targeted compensation for this range is $145K-$165K per year. New York Pharmacist License Required.

Responsibilities

ESSENTIAL FUNCTIONS (These duties must be performed with or without reasonable accommodation. This document in no way states or implies that these are the only duties to be performed by the employee occupying this position):

1. Deliver RxCare services, including but not limited to, Comprehensive Medication Reviews, Disease Specific Medication Reviews, and Targeted Interventions to assess appropriateness of all medications, to optimize medication therapy and to improve clinical outcomes.
2. Document clinical encounter in electronic medical record, including medication reconciliation and referrals.
3. Communicate and document clinical recommendations to prescribers and interdisciplinary care teams.
4. Produce presentations and reports to be shared with senior leadership and/or externally.
5. Lead analysis and generation of insights related to specialty drug trends.
6. Collaborate with specialty pharmacy, managed care, health systems and external clients
7. Develop and maintain clinical patient education resources
8. Promptly report any allegations of impropriety to the Compliance Department.
9. Comply with VytlOne's Ethical Business Conduct policy and VytlOne's Compliance Program.
10. Remain free from exclusion under the OIG and SAMS Medicare/Medicaid lists.
11. Complete required training, as assigned, within the established timeframes.
12. Must be able to cope with the mental and emotional stress of the position.
13. Perform other job-related duties as assigned.

Qualifications

QUALIFICATIONS:

Education:
• Bachelor's degree in pharmacy from a college accredited by ACPE. PharmD preferred
• Current license to practice Pharmacy in the state(s) you are practicing in addition to maintaining additional state licensures .

Experience:
• 5 years related work experience required in specialty pharmacy or related clinical services
• Demonstrated knowledge of pharmaceutical care, electronic medical records, pharmacy systems and design of clinical programs

Knowledge, Skills, and Abilities:
• Knowledge and understanding of Medication Therapy Review - Comprehensive and Targeted, Personal Medication List (PML), Medication-Related Action Plan (MAP), Interventions and Referrals, and Documentation of Services and Follow-up
• Ability to efficiently assess prescribed medication therapy for unnecessary medications, need for additional medications, ineffective medications, inappropriate doses, adverse drug reactions, and adherence concerns
• Ability to efficiently propose appropriate interventions to resolve medication therapy problems, achieve goals of therapy and prevent new medication therapy problems
• Ability to assist with managing medication costs through cost-effective interventions
• Ability to help patients identify medication adherence barriers and develop solutions to overcome those barriers
• Ability to efficiently evaluate and accurately document patient care notes, prior adverse reactions, patient recommendations, and prescriber recommendations in a clear, concise, professional and timely manner
• Ability to collaborate with prescribers, interdisciplinary care teams, patients and/or caregivers to identify and develop patient-specific disease state therapy goals
• Ability to provide patient-centered education on chronic disease states, including medications used for prevention and treatment
• Ability to speak clearly and communicate fluidly with senior citizens and healthcare professionals alike
• Ability to convey warmth and empathy to others while patiently employing active listening technique
• Ability to articulate complex information, both verbally and in writing, in a clear and concise professional manner to a variety of audiences
• Consistent demonstration of a professional attitude and behavior
• Ability to probe and question patients to extract information to understand concerns and needs
• Ability to defuse potential customer conflict and respond supportively to patients through use of appropriate tone, questioning, and/or counsel
• Ability to follow all company and departmental processes and procedures
• Self-motivation, organization, and attention to detail
• Ability to efficiently navigate operating platforms
• Proficient in use of MS Office applications, including Excel, Word, and Outlook
• Ability to multi-task using multiple databases/platforms/applications
• Strong clinical therapeutic skills and demonstration of knowledge of appropriate medication dosing in geriatric and adult populations
• Proven ability to work independently in a collaborative environment, be self-directed and possess excellent organizational skills
